Well, I got stupid and took the plunge. An upgrade from 3.8.4 to 4.1.0 PL2
The test run on my test site worked fine. And then when I ran the upgrade script on my live site, it crashed and burned. Goodness only knows why.
Fortunately I'd pulled a fresh copy of the live site db for the test site upgrade attempt. Just dropped all the folders and files from the test site into the live site area, switched the live site to the backup db and mission accomplished. I only lost a few hours of posts at the quietest time of the year, so no trainsmash.
My tips are:
back up
assume the upgrade will fail
back up
disable every product and plug-in
back up
make sure you know how to use bigdump
back up
test your ability to restore by setting up a test site
backup
check that your test site is working properly
backup
change your default skin to default vB
backup
upgrade the test site
backup
if that works, either flip the test site up to your live site like I did, or try your luck one more time on the live site.
Have a stiff drink to calm your shattered nerves.
This is not an upgrade to be tackled lightly. Do it on your live site without a backup handy at your peril.
Seriously.

You haven't looked hard enough.
The bugs... oh the bugs.
The stupidist, most visibly obvious bugs I've ever seen. The transition from 3.6.x to 3.7 Gold (which for the record wasn't a smooth ride either) was nothing compared to this.
Quite clearly there has been one big downside to the new licencing method and the controversy it created - there aren't enough people doing beta version testing anymore. I'm sure with more folk involved with beta testing, most of the bugs ironed out with the release of 4.1.1 Beta wouldn't have got into 4.1.0.
