Hi i used to have my register rules template a certain way and i dont no how they did it

basicaly see that
the user ticks the box, and they enter there email add n pass and then they click the button it will invite users to the forum and proceed
im not asking for the script as i have it,just to make the template look like that and submit to next reg form to,could anyone help me
