Wanted (Banner Designer)

hello all,
I'm looking for someone to create a custom banner and a default avatar for one of my other site, its very simple. Attached i will show how i want it .

Now for the banner the size i need is 468x60 and the default avatar will be 120x120 i want to use the same color as the attached one with mobile phone back ground.
